We keep the old state name, the old office name and the old stamp exactly as printed, then add a short note saying which country that town sits in today. Without that note a reader here simply sees a name they cannot place, and the file stops.

We do not quietly swap in the modern name, because your page and your passport would then disagree. We also do not leave the old name alone with no help, because nobody here would know what to do with it. For Singapore, yes. That sheet was made so European countries could pass papers to each other without a translator, and it works very well inside Europe. Outside it, offices have no rule that recognises it. An ICA or MOM counter will still ask for a full English translation with a signature on it. Keep the sheet, since it is useful if your file later goes back to Europe, but do not count on it here.
